How ongoing care will work
What happens once your treatment is settled: who prescribes, what the monthly plan covers, and how to move to your GP.

Membership is monthly with no minimum term. Cancel any time before your next payment date, online or by email. It does not guarantee that a prescription will be issued: prescribing is always a clinical decision made at your review, and the plan pays for the care around it.

After titration, your ongoing care follows one of these. We tell you which is likely for your area before you pay for anything.

- A monthly plan with no minimum term. Medicines are paid for at the pharmacy.
You can switch from route 2 to route 1 at any time. We can ask your GP again, and if they accept, you can end membership that month.

What membership includes
Everything for route 2 in one monthly plan.
- Regular reviews With your prescriber, at the interval agreed for you, plus a full annual review.
- Monitoring Blood pressure, pulse and weight checks on a set schedule, at home or at a pharmacy, recorded in your notes.
- Prescriptions Issued by your prescriber where clinically appropriate, and posted to the pharmacy you choose. The medicines are paid for at the pharmacy and are not included in the plan.
- Messages A named contact for questions between appointments, with a stated response time.
- Monthly community call An hour online, once a month. A short topic, then questions. You do not have to speak. Standard with coaching also adds a one-to-one coaching session each month.
- Shared care is still an option We can re-request shared care from your GP at any point, and if they accept, you can end membership that month.
How the costs work, from assessment to ongoing care
- Assessment A one-off fee. Includes the pre-assessment questionnaires, the structured assessment, a check of every assessment by our consultant psychiatrist, a results appointment, a written report, and a letter to your GP if you want one. The fee is the same whether or not the outcome is a diagnosis.
- Titration A fixed fee for the period from starting medication to a stable dose, usually eight to twelve weeks. Includes the starting appointment, monthly titration appointments, monitoring, and the prescriptions issued during that time. The medicines themselves are dispensed and paid for at the pharmacy, at the pharmacy's price.
- Ongoing care Nothing more if your GP takes over (route 1), or a monthly plan if you stay with us (route 2).
What is never included
The cost of the medicines (paid at the pharmacy), pharmacy delivery charges, and anything we have not listed.

How is membership paid?
Monthly by Direct Debit. You can cancel any time before your next payment.
Is this a subscription for medication?
No. It's a plan for the clinical care and support around your treatment. Medication is prescribed only where a clinician decides it's right for you at that review.
